United States

  • Allergy Alert: Completely Fresh Foods Inc. (Montebello, CA) recalls approximately 131,000 pounds of Trader Joe's Pizza Al Pollo Asado, 10-oz. pkg (Produced on various dates between jan 27, 2011 and March 27, 2011), due to the presence of undeclared wheat. The recalled pizza products were supplied to Trader Joe's stores nationwide.

  • Food Safety Recall: Nutrition Express (Torrance, CA) recalls specific products containing whey protein isolate from one supplier, because the products may be contaminated with Salmonella. Please see the recall notice for details.

  • Food Safety Recall: The Smokehouse LLC (Mamaroneck, NY) recalls PORTIER FINE FOODS Norwegian Smoked Salmon (various size packages; Batch Code 066) due to potential contamination with Listeria monocytogenes. The recalled Smoked Salmon was distributed and sold on March 7, 2011 to retailers in New York, New Jersey and Connecticut.

  • Food Safety Recall (Ohio): Pettisville Meats Inc. (Est 1240; Pettisville, OH) recalls various meat and poultry products that were produced without the benefit of state inspection and were sold prior at various carry-out and food service establishments in Ohio. Please see the recall notice for details.

  • Food Safety Recall: Price Chopper recalls Price Chopper Homestyle Regular Potato Salad (UPC 26716700000), due to possible contamination with Listeria.

  • Food Recall/Withdrawal: Food City announces the recall/withdrawal of Food Club Frozen Onion Rings and of Valu Time RTS Milk Chocolate Frosting. No details have been provided.

  • Parenteral Nutrition Product Safety Recall/Investigation: Meds IV recalls all of its intravenous (IV) compounded products. Total parenteral nutrition (TPN) products produced by Meds IV have been identified as a potential common source for 19 cases of Serratia marcescens bacteremia reported in six Alabama hospitals.

Canada

  • Allergy Alert: Quality Natural Foods Ltd. recalls Quality brand Freshly Minced Ginger Garlic, Freshly Minced Garlic, and Freshly Minced Ginger (Lot Codes L-210610, L-230910 and L-120410) due to the presence of undeclared sulphites. The recalled products were distributed in Alberta, British Columbia, Ontario and Quebec.


Europe

  • Food Safety Recall (Ireland): Burren Smokehouse Ltd. recalls Burren Smokehouse Smoked Salmon (1kg, 500g, 300g, 200g and 100g packs; Batch code 1018X; Use by 15/04/2011), due to the presence of low levels of Listeria monocytogenes. This is in addition to an earlier recall.

  • Food Safety Recall (UK): Premier Foods recalls Fray Bentos 'Boozy' Steak & Ale Pie, 475g (Batch code 1003 & 1004; Best before end January 2013) and Fray Bentos Steak & Onion Pie, 425g (Batch code 1004; Best before end January 2013), because the canned pies may contain small pieces of glass.


Australia and New Zealand

  • Allergy Alert (New Zealand): Lindt and Sprüngli recalls various Lindt chocolate easter eggs (Product of USA; Best before 30 JUN 11) due to the presence of undeclared peanuts. The 16g Easter Eggs are foil-wrapped and available in various package sizes and formats. The recalled confectionery products were sold in supermarkets and retail outlets nationwide.

  • Allergy Alert: Barry Callebaut USA LLC recalls Kroger Value Semi Sweet Chocolate Chips, 12 oz. (Best by dates: November 20, 2012, December 1, 2012, December 3, 2012 and December 4, 2012) because the product may contain undeclared milk protein. The recalled chocolate chips were sold in Kroger-owned stores, including Kroger, Dillon's, Gerbes, Baker's, Jay C, Hilander, Owen's, Pay Less, Scott's, Fred Meyer, Fry's King Soopers, Smith's, City Market, Foods Co., and Food 4 Less.
  • Food Recall: Price Chopper recalls Price Chopper 80% Ground Chuck (Sell by Jan 11) and Price Chopper 85% Ground Round (Jan 11), due to the possibility that a plastic utensil was ground into the beef. The recalled ground beef was sold at the Glen St. Price Chopper (Queensbury, NY) on Jan 11, between 1pm and 7:30 pm.


Canada
  • Alcohol Swab Alert: Health Canada advises Canadians that COPAXONE®, used in the treatment of Multiple Sclerosis, has been co-packaged in a kit produced by Teva with recalled alcohol prep pads manufactured by Triad. Teva is advising patients to continue their daily injection routine with Copaxone prefilled syringes, but to use alternative alcohol pads or bottled isopropyl alcohol (70%) with a cotton ball.


Europe


Asia, Africa and the Pacific Islands
  • Food Safety Alert (China): China State Council's Food Safety Commission has reported that a total of 96 people are in police custody, awaiting prosecution, or in prison for illegally using melamine-tainted milk powder. This disclosure comes on the heels of a further crackdown (in process since July 2010) on the use of tainted milk powder.


Australia and New Zealand
  • Food Safety Recall (New Zealand): Living Foods recalls Pams Fresh Express Baby Spinach, Pams Fresh Cos Mix Salads, and Living Foods Baby Spinach Salads (Best before 12/01/2011 up to and including 19/01/2011), because Listeria was detected in the product. The recalled spinach-based salads were available through New World, PaknSave, 4 Square, Gilmours and independent retail fruit and vegetable shops and wholesalers throughout New Zealand.

United States
  • Food Safety Recall: Autentico Foods (Hawaiian Gardens, CA) recalls approximately 54,000 pounds of ready-to-eat fried pork products and pickled pork skins that it did not produce in accordance with its food safety plan, failing to take the steps it had determined were necessary to produce safe products. The problem was discovered through a review of company records by FSIS in-plant inspection personnel. The review uncovered evidence to show that the establishment’s food safety records cannot be relied upon to document compliance with its Listeria prerequisite program. The fried pork products were distributed to retail establishments in California. The pickled pork skin products were distributed to retail establishments in California, Colorado, Florida, Idaho, Nevada, New York, Oklahoma, Texas, and Utah.


Canada
  • Homeopathic Product Safety Recall: Hyland's Homeopathic Canada (Hyland's), a division of Standard Homeopathic Company, recalls its Hyland's Teething Tablets from the Canadian market because they may pose a risk to children. Hyland's Teething Tablets are manufactured to contain a small amount of belladonna, a substance that can cause serious harm at larger doses. The US FDA has received reports of serious adverse events in children taking this product that are consistent with belladonna toxicity but conclusive links have not been formally determined.


Europe
  • Food Safety Recall Update (UK): The UK Food Standards Agency has issued a Food Alert for Action, asking local authorities to make sure that all batches of Sokolów's Home Black Pudding Sausages and Sokolów's Grill Black Pudding Sausages have been withdrawn from sale and recalled from consumers. The recall was initiated after the products were found to be contaminated with Listeria monocytogenes.
  • Food Safety Recall: Pentik Ltd. recalls jouluglögi (Christmas mulled wine), imported from Sweden (best before 28/02/2012), because of an excessive level of benzoic acid and errors in the product labeling.
  • Pharmaceutical Product Safety Recall (UK): Novo Nordisk A/S recalls GlucaGen® HypoKit, 1mg (Batch #YW60411 and YW60452) due to a defect in a small number of vials. In some vials, the glass of the powder vial is cracked close to the base so that when the solvent is added, the vial leaks.
  • Food Safety Notification (EU #2010.1458): Undeclared sulphite in dried figs from the United States, packaged in the United Kingdom; distributed to Greece.
  • Food Safety Notification (EU #2010.1459): Salmonella in chilled pork meat from Germany; distributed to Italy.
  • Food Safety Notification (EU #2010.1460): Pseudomonas aeruginosa in mineral water from Lithuania; distributed to Ireland.
  • Food Safety Notification (EU #2010.1461): Salmonella spp. in raw poultry sausages from Latvia; distributed to Estonia.
  • Food Safety Notification (EU #2010.1462): Stones in frozen forest fruits from Belgium; distributed to Denmark.
  • Allergy Alert Notification (EU #2010.1463): Undeclared soya in meatloaf from Poland; distributed to Slovakia.
  • Food Safety Notification (EU #2010.1464): Metal fragments in poultry meat sausage from Germany; distributed to Germany.
  • Food Safety Notification (EU #2010.1465): Listeria monocytogenes in ham from Germany; distributed to Slovenia.
  • Consumer Product Safety Notification (EU #2010.1466): Migration of primary aromatic amines from black nylon spoons from China; distributed to France.
  • Food Safety Notification (EU #2010.1467): Listeria monocytogenes in smoked salmon from Lithuania, via Germany; distributed to Austria.
  • Animal Feed Safety Notification (EU #2010.1468): Salmonella group C in extracted soya bean meal from Argentina; distributed to the Czech Republic and Germany.
  • Food Safety Notification (EU #2010.1469): Presence of poisonous mushrooms - Amanita phalloides in fresh mushrooms from Bulgaria; distributed to Italy.
  • Food Safety Notification (EU #2010.1470): Ochratoxin A in ground corn for tortillas from the United States; distributed to the United Kingdom.


Asia, Africa and the Pacific Islands
  • Dietary Supplement Safety Alert (Hong Kong): The Department of Health warns members of the public not to buy or consume a slimming product named “Crystal Pills” as it was found to contain undeclared sibutramine, a western drug ingredient that may be dangerous to health.
  • Proprietary Medicine Recall (Hong Kong and Macao): Hong Kong Medicine Manufactory recalls Wintex Tong Hang Wan Pills (Batch #PN0702W1) after the Macao Health Bureau detects excessive amounts of yeast and mold in the product.
  • Contact Lens Safety Recall (Hong Kong): Johnson & Johnson (HK) Limited extends its August 19th recall of 1 Day Acuvue TruEye contact lenses to include additional lot numbers. The product recall now encompasses lot numbers falling between 492237 and 492498 and between 502080 and 502269. According to Johnson & Johnson, detailed investigation into the incident in August revealed that the lens rinsing process on two production lines did not meet the internal production standards of the company, leading to a residual trace amount of diluent in the lenses.


Australia and New Zealand
  • Food Safety Recall (Australia): Moti Group Australia recalls Moti Angel Coffee weight loss beverage, 50g and 100g cardboard boxes containing 10 x 5g and 20 x 5g packets, respectively (all expiry dates up to and including 25/09/2010), because the product contains sibutramine, a non-compliant prescription medicine. The recalled product was imported from Hong Kong and distributed only in New South Wales.
  • Cosmetic Product Safety Recall (Australia): Beauty Secrecy Corp Pty Ltd (T/A. Locks Lash) recalls all batches of Locks Lash - Jet Lash Adhesive (eyelash extension glue), due to the presence of excessive concentrations of formaldehyde. The recalled product was sold in New South Wales, Queensland, Victoria and Western Australia through Sunstate Therapy Supplies, Budget Beauty Supplies, Capital Salon Supplies, and Casey College of Beauty.

United States
  • Norton, KS. September 11th: The Kansas Department of Health and Environment rescinded the boil water advisory for the City of Norton, after lab testing on samples collected indicated no evidence of contamination.
  • San Diego County, CA. September 14th: The County of San Diego, Department of Environmental Health has issued a Boil Water Order and Public Notification for the Wynola Water District (Julian), because the drinking water system has tested positive for the presence of total coliform bacteria (and absent for E. coli). The Boil Water Order will remain in effect until the distribution system has been disinfected and additional samples confirm the absence of bacteria in the water supply.


Europe
  • United Kingdom. September 15th: Research by the Health Protection Agency (HPA) has revealed that over half the cleaning cloths used in restaurant and take-away kitchens were found to contain unsatisfactory levels of bacteria which are a sign of poor hygiene and cross contamination. The research team sampled 133 cloths, from 120 establishments in the North east of England, and found that 56 per cent contained unacceptable levels of bacteria. The most common of which were Enterobacteriaceae (found on 86 cloths), E. coli (21 cloths), Staphylococcus aureus (six cloths) and Listeria (five cloths).
  • Ulyanovsk, Russia. September 15th: Thirteen children (age 4 to 6 years) who attend a nursery in the village of October Cherdaklinskogo fell ill recently with vomiting and fever. They were taken to hospital, where they were diagnosed with an intestinal infection. Two remain hospitalized. Based on preliminary investigation, the source of the infection is suspected to be either kefir or watermelon, both of which were on the kindergarten menu that day.


Asia, Africa and the Pacific Islands
  • Mascara, Algeria. September 11th: Six members of one family were admitted to hospital on September 7th complaining of vomiting and acute diarrhea. One of the victims – a 30-year old woman – died at home after leaving the hospital in the company of another family member on September 10th. An autopsy has been ordered. Three people, including two children, remain under medical observation.
  • Sidi Brahim, Algeria. September 14th: A flock of 8,500 broiler chickens was ordered slaughtered after lab analysis determined that the flock was infected with Salmonella Enteritidis. Steps have been taken to disinfect the equipment and transport vehicles which came into contact with the infected chickens.
  • Taiwan. September 14th (update): Taiwan Centers for Disease Control reports an imported outbreak of shigellosis. A life insurance company invited 140 members of the company to Siem Reap, Cambodia, during 3-7 Sep 2010. Upon the group's return through Taoyuan International Airport, Taiwan, on 7 Sep 2010, quarantine officers found 7 members with fever and diarrhea. The earliest onset of symptoms was on 6 Sep 2010. Rectal swabs were taken from 7 persons. On 10 Sep 2010, 4 were confirmed to be infected with Shigella flexneri. Contact tracing of all members of the tour group found an additional 51 persons with symptoms.
  • Taiwan. September 15th: In April, 113 agricultural products were randomly sampled and tested in the first testing, of which 102 were found in compliance with relevant regulations (qualified rate 90.3%). The agricultural products that failed the tests included: water spinach (2), cucumber (1), egg plant (2), bell pepper (1), tomato (1), snap bean (2), kidney bean (1), and cucumis sativus (1).
  • Harare, Zimbabwe. September 15th: A fresh outbreak of cholera has killed one person and infected four others in Chiredzi town, some 300 kilometers north of Zimbabwe's border with South Africa.


Australia and New Zealand
  • Canterbury, NZ. September 15th: The New Zealand Food Safety Authority is warning cafés, restaurants and whitebaiters that food taken from sewage-contaminated rivers and estuaries could cause serious illness. The Heathcote, Avon, Halswell and Stxy Rivers are probably contaminated with sewage, according to the warning.

United States
  • Food Safety Recall: Midland Farms (Menands, NY) recalls certain milk products (date code SEP24 or SELLBY 9/24/10), after a September 7th inspection by New York State Department of Agriculture and Markets' milk inspectors found a failure of pasteurization equipment at the milk plant. The recall includes gallon, half-gallon, quart and pint-sized plastic containers of milk, sold under the Midland Farms, Corrado's Market, Jersey Dairy Farms and Trade Fair Premium brands. All of the milk containers are marked with plant code 36-1661, and a black ink date code near the top of the container. The recalled milk products were sold in New York, Connecticut, Massachusetts, New Jersey and Rhode Island.


Canada
  • Food Safety Recall: Leadbetters Foods Inc. (Orillia, ON) recalls Leadbetters Cowboy Beef Burgers, because the product may be contaminated with E. coli O157:H7. The burgers are sold frozen in 2.27Kg (5 lb) cartons containing 20 burgers (UPC 8 73587 00003 5; code 20169). The recalled beef burgers were sold only in Ontario. Four confirmed cases of E. coli O157:H7 illness have been linked to this meat.


Europe
  • Food Safety Withdrawal (UK): Ella's Kitchen withdraws certain batches and date codes of Ella's Kitchen Stage 1 baby food and The Yellow One Smoothie Fruit because of possible microbial spoilage inside the straw of the packets. None of the microorganisms that have been identified are pathogenic.
  • Food Safety Recall (Denmark): Dansk Supermarked recalls Loving turkey (900g pack; produced 23/08/2010; expiry 04.9.2010), imported from Germany, because Salmonella was found in a sample of the turkey breast. The recalled product was sold in certain Net stores in Denmark (follow the live link for a list of affected stores).
  • Food Safety Recall (Denmark): JN MEAT Aps recalls smoked beef tenderloin (produced 26-08-2010; expiry date 30-09-2010), after Listeria is found in a sample of the product. The recalled beef was sold to canteens and caterers.
  • Food Safety Notification (EU #2010.1214): Algae in spring water from Turkey; distributed to Denmark.
  • Animal Feed Safety Notification (EU #2010.1215): Salmonella in processed animal protein from the Netherlands; distributed to the Netherlands.
  • Consumer Product Safety Notification (EU #2010.1216): Migration of chromium from steak knives set from Brazil; distributed to Slovenia.
  • Consumer Product Safety Notification (EU #2010.1217): Migration of cadmium and of lead from glasses from China; distributed to Hungary and Slovenia.
  • Allergy Alert Notification (EU #2010.1218): Undeclared egg and traces of peanut in cake from Lithuania; distributed to Ireland.
  • Food Safety Notification (EU #2010.1219): Excessive nitrate content in spinach from the Netherlands; distributed to the Czech Republic and Slovakia.
  • Food Safety Notification (EU #2010.1220): Unauthorised enrofloxacin in eggs from Poland; distributed to the Czech Republic, Poland, Slovakia and the United Kingdom.
  • Food Safety Notification (EU #2010.1221): Parasitic infestation with Anisakis simplex of chilled angler (Lophius piscatorius) from the United Kingdom; distributed to Italy.

  • Allergy Alert: Rich Products Corporation recalls Allen Bavarian Crème Filling (product code 02881) with production codes 11870137F21, F-22, F-23, F-24, F-25, because the product may contain undeclared pecans, milk, and coconut.
  • Dietary Supplement Safety Alert: FDA warns consumers that Magic Power Coffee, an instant coffee product marketed as a dietary supplement for sexual enhancement, contains hydroxythiohomosildenafil, an active drug ingredient that can dangerously lower blood pressure.

  • Counterfeit Pharmaceutical Product Alert (Australia): The Therapeutic Goods Administration is urgently investigating the source of the counterfeit Viagra (sildenafil) that has been supplied in Australia and has entered the wholesale/pharmacy supply chain. The counterfeit product is labelled as 100 mg tablets in packs of 4 with Batch Number 314833021 and Expiry Date 04 2012. Lab analyses have shown that the active ingredient, sildenafil, is present in an incorrect dose.
